Acknowledgment of Risk:
I, the undersigned, acknowledge that participation in the Kid Nations Ministry at Save the Nations Church may involve physical activities, crafts, games, outdoor play, and other events that may carry inherent risks, including but not limited to physical injury, illness, or harm. I voluntarily allow my child/ward to participate in these activities with full understanding of the potential risks involved.
Waiver of Liability:
I hereby release, discharge, and hold harmless Save the Nations Church, its employees, volunteers, leaders, and affiliates from any and all liability, claims, demands, or actions, which may arise from participation in any Kid Nations Ministry activities. This includes any incidents caused by negligence, except for incidents of gross negligence or willful misconduct.
Medical Treatment Authorization:
In the event of an emergency, I give my consent for medical treatment to be provided to my child/ward if I cannot be reached. I understand that I will be responsible for any medical expenses incurred, and I agree that Save the Nations Church is not responsible for the costs of such treatment.
Media Release:
I grant permission for Save the Nations Church to use photographs, video footage, or audio recordings of my child/ward participating in Kid Nations Ministry activities for promotional or informational purposes, without compensation.
Code of Conduct:
I understand that my child/ward is expected to follow all rules and instructions provided by Kid Nations Ministry staff and volunteers. Failure to adhere to the rules may result in the removal of my child/ward from the activities.
Governing Law:
This waiver shall be governed by the laws of the State of Florida, and any disputes arising under this agreement shall be subject to the jurisdiction of the courts in Broward County, Florida.
